You can obtain a White Card once you have been accepted for an occupational illness or a covered illness under the Energy Employees Occupational Illness Compensation Program Act (EEOICPA). The EEOICPA White Card, as it is commonly referred to by claimants, is a Medical Benefits ID Card issued by the Division of Employee Occupational Illness Compensation’s (DEEOIC) bill pay agent.
In order to obtain a White Card you must file a claim under the EEOICPA and be awarded medical benefits for either an occupational illness or a covered illness.
A new White Card can be issued by the DEEOIC’s bill pay agent if you lost your current White Card. To have a new White Card or Medical Benefits ID Card issued contact the DEEOIC bill pay agent at: 1-866-272-2682. Once you have reached a customer service agent simply request a new card. Be prepared to give your file number, date of birth, and confirm your mailing address.
